Become a Mentor:

The most important qualification for a Mentor is to want to help Consumers effectively navigate the IHSS system and to support them in maintaining their independence in the community of their choice. Mentors achieve this by coaching Consumers through finding, hiring and working with their Provider of choice and assisting Consumers in connecting with other services that help them meet other essential needs.

Community Engagement Coordinator Posted on April 17, 2024

The Public Authority’s Mentorship Program takes a solutions-based approach and serves Consumers within a culture of empathy and encouragement. The Program helps Consumers receiving IHSS PA services navigate the IHSS system, find stable home care providers, and get connected to other community resources to promote and support independent living throughout San Francisco. The Community Engagement Coordinator works directly with Skilled Nursing Facilities and other institutional settings to provide information and guidance to facility staff and community based service providers, while collaborating with the Consumer to create a plan of action for the transition from institutional care to home care with IHSS PA and additional community supports in place. The Community Engagement Coordinator assesses Consumer needs and creates an individualized support plan. Once the support plan is in place, a Mentor is assigned to work with the Coordinator and Consumer to initiate service.  The Coordinator continuously monitors service progress, recommends improvements, schedules home visits to ensure the Consumers’ needs are being met, and maintains service case records.  This position is part of a team of full-time and part-time staff and Mentors who ensure that IHSS PA Consumers receive efficient responses, are heard, have their questions answered, and are supported throughout the process of engaging IHSS PA service.… Learn More & Apply Online

Timely documentation of all communications regarding consumer care plans and service needs in Salesforce. Provide supportive coaching and information to consumers and home care providers navigating the IHSS system. Effectively communicate with consumers, their representatives, home care providers, office staff, management, business partners, and the public. Clearly explain services verbally and in written form, ensuring all parties have a clear understanding of expectations. Accurately enter provider and consumer information into the Salesforce database. Maintain confidentiality of case files through proper documentation and case notes. Conduct follow-up on consumer and provider matches to ensure satisfaction and address any concerns or issues. Investigate and process consumer and home care provider complaints according to established Policies and Procedures. Conduct consumer satisfaction surveys and record responses in Salesforce. Identify top performing Registry Providers and recommend them to the Recruitment team as potential on-call providers.
