DAAS case management helps older persons, adults with disabilities, or their caregivers coordinate all the services that they may need in order live in their homes or care facilities. The case manager may assess the client's needs, develop a care plan, authorize and arrange for services, coordinate the provision of services among providers, monitor care, and help with discharge from care facilities. There is a waitlist. Eligibility is restricted to San Francisco residents who are 60 years of age or older, or individuals between 18 and 59 years of age who are living with disabilities, who are at risk of premature institutionalization and who meet at least one of the following conditions:

• impairment in one or more areas of activities of daily living, such as eating, dressing, transfer, bathing, toileting, and grooming.

• impairment in two or more Instrumental Activities of Daily Living, such as taking medications, stair climbing, mobility indoor, mobility outdoor, housework, laundry , shopping and errands, meal preparation and cleanup, transportation, telephone, and money management

• unable to manage his/her own affairs due to emotional and/or cognitive impairment.

• impaired by virtue of a significant event or circumstance that has occurred within the past 12 months
