Enrollment begins with Self-Help for the Elderly, 731 Sansome St Site 100 or online at www.sfchampss.org/enroll/. Eligibility restricted to individuals 60 and over unless individuals under 60 are in specific housing sites with its own meal site. The process takes approximately two months. After enrollment, a card (similar to a credit card) will be mailed to the Consumer. The card can be used at two restaurants:

Weekday Service: Monday, Wednesday, Thursday & Friday
