The Affordable Care Act (ACA) requires people to have Minimum Essential Coverage (MEC) health insurance. This includes coverage through the Healthy Workers program. Healthy Workers is a health insurance offered to San Francisco IHSS independent providers (IP’s) who work a minimum of 25 hours every month. If you were enrolled in the Healthy Workers program in 2016, you should receive a 1095-B tax form from the San Francisco Health Plan by March 2, 2017. This form will list the months that you were covered under the Healthy Workers program. You can use this as a proof of coverage when you file your 2016 tax return.

If you do not receive your 1095-B tax form by March 2, 2017, please contact the San Francisco Health Plan at 1-800-288-5555 or the San Francisco IHSS Public Authority at 415-593-8125. If you want to enroll in the Healthy Workers program, please contact the Benefits Coordinator at 415-593-8125 to check your eligibility and obtain an enrollment packet.
